How to Request Medical Records
You can request your records by:
Completing a Medical Records Release Form (available online or in our office)
Submitting your form via fax, mail, or in person
Allowing a short processing time (usually 3–5 business days)
We’ll notify you when your request is complete and provide your records in the format you choose—print or digital.



Who Can Request Records
You, the patient
A parent or legal guardian (for minors)
A medical power of attorney
A provider or facility (with signed patient authorization)
