Whether you're a new patient, coming in for a referral, or scheduling a work-related injury visit, we’ve made it easy to get started.


Filling out your paperwork ahead of time allows our team to review your information, verify insurance, and get you seen faster.

Downloadable Forms

New Patient Registration Packet

This form includes your contact info, insurance, medical history, and treatment consent. Please fill it out completely before your first visit.

Referral Form

For referring providers. This form helps us process patient referrals smoothly and schedule based on location and physician preference. Please include any relevant notes or imaging.

Workers' Compensation Verification Form

If your injury is work-related, this form must be completed by your employer prior to your appointment. It verifies your eligibility for workers’ comp coverage and helps us coordinate care and claims.

How to Submit Forms

You can return your completed forms in one of the following ways:

  • Email them to our office staff (call for correct email based on your location)

  • Fax them to 985-625-2206

  • Drop them off in person before your appointment

  • Bring them with you on the day of your visit
     

We ask that new patients submit their forms at least one day in advance when possible.
