Whether you’re using private insurance, Medicare, or a workers’ compensation plan, we’re ready to guide you.

How to Submit Forms

OrthoLA is a participating provider with most major insurance plans. We work directly with your insurance company to file claims and coordinate benefits. We currently accept:

  • Aetna Health

  • Blue Cross Blue Shield (HMO, PPO, POS, and Office of Group Benefits)

  • Cigna

  • Coventry

  • Gilsbar

  • Humana and Humana 65

  • Medicare

  • Multiplan

  • People’s Health Network

  • PPO Plus

  • Private Health Care Systems (PHCS)

  • Tricare Standard and Tricare for Life

  • United Health Care (Commercial and Medicare Alternative Plans)

Workers’ Compensation & Injury Programs

We also participate in a wide range of programs for injured workers, including:

  • Louisiana Workers’ Compensation

  • Longshore Harbor Workers

  • LWCC

  • Jones Act

  • OWCP (Federal Workers’ Compensation)

If your injury is job-related, please notify our team before your appointment so we can ensure the proper paperwork and authorizations are in place.

Out-of-Network Insurance

If we are not listed as an in-network provider for your plan, out-of-network charges may apply. Call our office and we’ll help you understand what’s covered and what your out-of-pocket costs might be.

Financial Responsibility

Even with insurance, patients are responsible for certain costs, including:

  • Deductibles

  • Co-payments

  • Co-insurance

  • Any non-covered services
     

Payment is due at the time of your visit. For your convenience, we accept:

  • Cash

  • Checks

  • Debit cards

  • Visa, MasterCard, and American Express
     

If you don’t have insurance, please call us to discuss your options before your appointment. We’ll do our best to help you plan ahead.

Helpful Tips for a Smooth Billing Experience

  • Bring your insurance card and photo ID to every appointment

  • Let us know about any changes in your insurance before your visit

  • Review your Explanation of Benefits (EOB) statements regularly

  • Pay outstanding balances before scheduling new appointments

  • Contact our billing department with any questions—especially about surgical charges or out-of-pocket costs
